Annual QSP Survey
The annual Qualified Service Provider (QSP) Survey is open. Thank you for serving as a QSP! We are glad you are part of the programs that allow people to live at home and in their community. We are looking for feedback about being a QSP and want your input! Please fill out the short survey about being a QSP agency, what could make it better, and what your direct service staff would like to see. We hope you will complete the survey and share your thoughts.
The web survey must be completed by the end of the day on July 27.
The survey takes about 10-15 minutes to complete.
