QSP Family Personal Care Enrollment

To apply as a Family Personal Care (FPC) Service Provider, you will need to access the following:

    This video walks you through the QSP Onboarding Orientation training needed for enrollment.
  • QSP Onboarding Training Requirements
    Duration: 5 minutes
    This video explains the steps and requirements needed to complete the QSP Onboarding Orientation training for enrollment.
  • Direct Deposit
    Duration: 4 minutes
    Direct deposit is one of the many application requirements. This video will go over the acceptable versions of direct deposit for your application or maintenance request.
  • Fraud, Waste, and Abuse Walk-Through
    Duration: 3 minutes
    This video walks you through the Fraud, Waste, and Abuse training that is required for all QSPs.
  • Fraud, Waste, and Abuse Requirements
    Duration: 3 minutes
    This video will explain the steps and requirements needed to complete the required Fraud, Waste, and Abuse (FWA) training that is required for all QSPs.

  • QSP Onboarding Orientation
    Complete this QSP orientation as a part of the onboarding and application process. You will need to enter your name and take a picture, screenshot, or save the last slide when the certificate shows you have completed the training. You will need to upload proof of this certificate directly into the application using the web portal.
